"THEY DISAPPEAR" is also incorrect.
"Perhaps I am only a nameless island on YOUR map? " - Yes! Now this line is completed.
"COULD" - Yes!
"the great earth/planet" - Earth in the sense of planet would work :) Although there's a better term that encompasses both words ;)
"MY momentS RUN" - "my moments" is correct! "Run" is very close. If you're doing a more poetic translation, it would be roughly the right meaning, as opposed to the literal meaning. But I think I can take this, or I can just wait and see if you change it at all once you're finished translating the rest.

Technically, you could have left "secrets" on the first line, because both "those thousand paths" and also "secrets" wait for you, but it is a correct translation.

Only one phrase remains to be corrected...
"wishing to leave"
Again, try to keep track of the subject, so tweak the verb a bit.
